4. By submitting this application, you authorize NU Packaging Inc to make inquiries into the banking credit and business/trade references that you have supplied.
5. $35.00 fee for returned checks will be added to your account.
6. For your convenience we accept Visa, MasterCard, American Express, and Discover. 3% Transaction Fee will be
added for any credit card payments after 10 days of invoice date.
